IMMUNIZATIONS

Immunization records are retained at our school only until you reach age 27 and are then destroyed. 

Please contact our office at 315-218-4003 to request a copy of these records. 

We recommend that you keep a copy of your immunizations with your birth certificate or other permanent records. 

If you are over 27 and your records have been destroyed, you may be able to retrieve some information from the NYS database:
