This page contains all materials and resources to help you successfully prepare and submit your video for the FND In Focus Annual Video Competition.

All information is available directly on this page, and downloadable versions are provided for participants who prefer printable or offline copies.

Competition Materials

Logos

Grab your logos here! There are four versions to choose from—the first has a transparent background, making it easy to add to your video.  

Just make sure to include at least one logo in your submission—it is one of the video requirements.

Please note: logos are for use in your video only.

Required Forms

Some submissions may require additional forms depending on who appears in, or is submitting, the video. Downloadable versions are provided below. 

All forms must be emailed with your video title as the subject line and/or in the body text. 

Important: Your video submission is not considered completed until all forms are fully completed, with signatures, and have been received.

Model Release Form

Every individual who appears in your video—whether visible on camera, heard speaking, or otherwise identifiable—must have their own signed Model Release Form. This allows Not Defined By FND to share and promote the video as part of the competition.

Parent / Guardian Consent Form

Participants under 18 years of age must have permission from a parent or legal guardian to participate.
