SPORT PHYSICALS for 2024-2025 ATHLETICS SEASONS
Forms for Sport Physicals are available:
*Online: click on District Web Page followed by Athletics, Forms
*Health Offices at Middle School and High School
*Athletics Office at High School
Every athlete needs a sport physical on file dated within the last 12 months of beginning the sport.
Please remember to keep a copy of your child’s current physical form for future reference, or to upload on ArbiterSports.
WE RECOMMEND YOU USE YOUR PRIVATE PHYSICIAN FOR THE SPORTS PHYSICAL.
If for some reason you cannot see your own physician, sport physicals will be performed by the school doctor, on the following dates and times:
High School Health Office
Thursday, May 23 – 12:00 PM
Thursday, June 6 – 12:00 PM
Middle School Health Office
Thursday, May 23 -- following high school physicals*
Thursday, June 6 -- following high school physicals*
*Athletes must see the nurse, Ms. Amorgianos, High School, or Ms. Petrucci, Middle School, to schedule the physical by the Medical Director. Each Athlete MUST also obtain a Health History Questionnaire, from the Health Office, that MUST be completed, signed by a parent and returned prior to the physical .
All paperwork for the High School Fall 2024 Sports Season is due on ArbiterSports no later than August 14th, 2024, but not before July 19th, 2024. Any questions, please call Mrs. Amorgianos or Mrs. Betzios at 277-7010.
All paperwork for the Middle School 7th and 8th grade Fall 2024 Sports Season is due on ArbiterSports, no later than August 23rd, 2024, but not before August 3rd, 2024, and. Any questions, please call Ms. Petrucci or Mrs. Betzios at 277-7310.

In order for your student athlete to be cleared, you MUST upload a current physical, and all health history questions must be answered accurately on FamilyID/ArbiterSports. Any discrepancies in the Health History questions, or failure to upload a current physical will result in a delay in your student athlete's sports clearance.
Following submission, it is important that you regularly check your primary email registered on ArbiterSports, as you may receive notice from the Health Office requesting additional information in order to complete your athlete's registration and clearance. All communication with the Health Office will only be via email. Keep in mind, that your student athlete will not be cleared until their application is complete.
If you have any difficulty uploading information to Arbiter Sports, please contact their Technical Support Staff using the telephone number/email address at the top of the ArbiterSports webpage.

Middle School Sports Announcement:
As we begin the Middle School fall sports season, the availability of game officials is once again a significant hurdle for the 2023-24 school year. Frankly, due to the extreme shortage of officials available to officiate all sports in general, Section VIII has a limited pool of officials to assign for Middle School sports. While this is certainly not an ideal situation, I can assure you that Section VIII and its governing body is working diligently to have as many Middle School games as possible assigned game officials. This will continue to remain a fluid situation for the foreseeable future.
That being said, there will be games on the current schedule that will not be assigned game officials. In these instances, our coaches do their best to both coach and maintain the safest possible playing environment for our student-athletes to participate in. These are considered “game scrimmages”. Please remember that we are attempting to deal with a rather unique situation, one that is not exclusive to just the North Shore CSD as this is occurring throughout all of Nassau and Suffolk County.
We are asking that you remain patient and supportive of all players and coaches as we again deal with this unusual situation for Middle School Sports. As always, we value your partnership and appreciate your continued support of our coaches and student-athletes in the North Shore community.

Tickets for the NS Athletics Varsity Dinner on Tuesday, June 6 at the Muttontown Club will only be available until Thursday, June 1 in the Athletics Office. No tickets will be sold at the door.
$58 per person except for Senior Athletes who are $20.

Dear Parents:
Going back many decades, the North Shore Athletics Department has had a great tradition of athletic excellence, and this year’s accomplishments have just added to those traditions.
We are extremely proud of those student athletes who have represented our teams, schools and community. On Tuesday, June 6, we will be honoring all our varsity teams and athletes for the entire year at the Muttontown Country Club. The program starts promptly at 6:00 PM.
We would like to extend an invitation to you and your child to share in this extremely memorable evening. All tickets can be purchased through the Athletics Office from Tuesday, May 22nd through Wednesday, May 31st. The cost of each ticket will be $58, with the exception of the seniors who will pay $20 with the remainder being paid by the North Shore Athletics Booster Club.
In order to allow sufficient time to complete the necessary arrangements, no tickets will be sold at the door. Therefore, please plan on purchasing your tickets as soon as possible as ticket sales are limited. All checks must be made payable to the North Shore Athletics Booster Club.
Your son’s/daughter’s attendance will make this an outstanding event. We look forward to seeing you at what promises to be a wonderful evening.

We have received some messages asking for "Sports Physical" clarification. Sports physicals and School Physicals are to be completed on the mandated New York State School Health Examination form (versions 2022 or 2023 are currently acceptable). Please tell your child's physician when a physical is for sports clearances.

SPORT PHYSICALS for 2023-2024 ATHLETICS SEASONS

Every athlete needs a sport physical on file datedwithin the last 12 months of beginning the sport.
Please remember to keep a copy of your child’s current physical form for future reference, or to upload on FamilyID.
WE RECOMMEND YOU USE YOUR PRIVATE PHYSICIAN FOR THE SPORTS PHYSICAL.
If for some reason you cannot see your own physician, sport physicals will be performed by the school doctor, on the following dates and times:
High School Health Office
Wednesday, May 24 -- 11:30 AM
Monday, June 5 -- 10:00 AM
Middle School Health Office
Wednesday, May 24 -- following high school physicals
Monday, June 5 -- following high school physicals
Athletes must see the nurse, Ms. Amorgianos, High School, or Ms. Petrucci, Middle School, to schedule the physical by the school doctor.
All paperwork for the High School Fall 2023 Sport Season is due on FamilyID no later than August 11th, 2023, but not before July 21st, 2023, for football and August 21st, 2023, but not before July 28th, 2023 for all other JV/Varsity Sports. Any questions, please call Mrs. Amorgianos or Mrs. Betzios at 277-7010.
