What type of training/schooling does the student want to pursue?

Is the chosen setting appropriate to the career outcomes the student has identified?

What accommodations might the student expect ?

What areas of life will the student need assistance navigating?

Does the student need continuing mental health service?

Are there other health needs that should be addressed?

In what type of community, and where does the student want to live

Does the student need help gaining independence?

(Financial planning, housing, transportation)

Identify Where and how the student wants to live (Alone, with family, with partners, etc)
