Transition Timeline For Students (With TEAM Input)
Age 14-15:
Begin to think about vocational and employment goals, explore what type of training you will need
Begin attending all IEP meetings and provide input about your goals
Begin developing independent living skills - cooking, budgeting, etc.
Learn about and be able to discuss your IEP accommodations and modifications
If applicable:
See that vocational training is part of your goals
Find out how funding sources (DHS, SSD, etc.) could benefit your child
Begin to identify long term needs and how to meet them (I.E. guardianship, independent living, etc.)
Age 16:
Explore Career Interest Inventories to help in identifying career options
Identify your personal learning style
Obtain legal ID - Drivers’ License or Non-drivers’ ID
Begin gaining employment experience through a part-time job or through volunteer work
Consider referral for Chapter 688 Transition Services
If Applicable:
Complete a vocational assessment
Begin to consider Graduation Options - Diploma, Certificate, Age out at 22
Begin the process of referring to Post High School programming
Begin the Guardianship process as well as any other legal processes (Health care proxy, etc.)
Age 17:
Begin to develop a specific career path with goals to help you achieve that path
Ensure that your IEP contains those accommodations that best meet your learning needs - Especially those that might be provided in college
Determine the type of college or vocational training program you wish to attend and begin the application process
Meet with the disabilities coordinator at your chosen training institution
Make Chapter 688 Referrals to relevant agencies
If Applicable:
Gather paperwork necessary to apply for SSDI, Medicaid, etc.
Begin to make arrangements for financial resources (Trusts, etc.)
Begin exploring with transition programs (independent living, vocational training, etc.) and apply to those relevant
Age 18:
Select IEP signing rights
Register to vote
Complete College/Vocational Training Application process
Develop a Resume - be sure to collect references (even from volunteer programs)
If Applicable:
Ensure that all Legal Processes - Guardianship, Healthcare Proxy, Financial Guardianship, Etc. have been completed
Apply for SSDI, Medicaid, Etc.
Begin transitioning to post high school programming
