Why do it?
For students with significant disabilities, a chapter 688 referral is a way to ensure that they receive the needed adult services upon graduation.
When to do it:
Referrals should be made two years before a student is expected to graduate or age out of high school
How to do it:
At a student's IEP meeting occurring two years before completing high school, the TEAM should discuss referral as part of the transition plan and if it is determined that a student is appropriate for the referral, the application should be completed
What are the benefits?
While students and families are able to apply directly to the various human services agencies, completing a chapter 688 referral will ensure that all agencies that might be relevant in a student's transition are included.
What agencies are included on the referral?
Massachusetts Rehabilitation Commission, Department of Children and Families, Department of Mental Health, Department of Developmental Services, Commission for the Blind, Commission for the Deaf and Hard of Hearing.
