Guidelines for OSHA 300 Form Recordable Cases

Recordable Cases
Record the case on the OSHA 300 form when a work-related injury or illness results in one or more of the following:
Death (Column G)
Complete (as opposed to partial) days away from work (other than the day of the injury or onset of illness) (Column H)
Restricted work activity (Column I)
Transfer to another job (Column I)
Medical treatment beyond first aid (Column J)
Loss of consciousness (Column J)
Significant injury or illness diagnosed by a licensed medical provider (Column J)
Hospitalization
Loss of hearing
Any stitches
Prescription medications
Non-prescriptions medications at prescription strengths
Staff who have a positive TST following a negative TST according to the Safety Manual: Prevention and Control of Tuberculosis (TB) Transmission Plan
Staff who have a case of MTB disease with previously documented negative TSTs or symptom screens according to the Safety Manual: Prevention and Control of Tuberculosis (TB) Transmission Plan
Non-Recordable Cases
Do NOT record the case on the OSHA 300 form when treatment consists ONLY of the following:
First aid (with no other treatment)
Diagnostics
A visit to the doctor’s office
Non-prescription medications at non-prescription strengths
Td administration
Cleaning, flushing, or soaking wounds on the skin surface
Using wound coverings, such as bandages, bandaids, gauze pads; use of SteriStrips or butterfly closures
Use of hot or cold therapy
Use of totally non-rigid means of support, such as an elastic bandage or wrap, non-rigid back belts
Use of temporary immobilization devices for transportation of accident victims, such as back boards, slings, neck braces, splints
Drilling a fingernail or toenail to relieve pressure
Draining of fluid from blisters
Use of eye patches
Use of simple irrigation to remove foreign bodies not embedded in the eye
Use of simple measures to remove splinters
Use of finger guards
Drinking fluids to relieve heat stress
Privacy Concern Cases
Privacy concern cases are recorded by case number only. Do not use the employee’s name. Email the employee’s name and case number to hr@nkyhealth.org.
The following injuries and illnesses are designated privacy concern cases:
An injury or illness to an intimate body part or the reproductive system
An injury or illness resulting from a sexual assault
Mental illness
HIV infection, hepatitis, or tuberculosis disease and TST conversions /infections (Do NOT record tuberculosis cases that originated prior to employment with the NKY Health.)
Needlestick injuries or cuts from sharp objects that are contaminated with another person’s blood or other potentially infectious material
Other illness, if the employee independently and voluntarily requests that his or her name not be entered on the log
For Privacy Concern cases, the head of Human Resources keeps a separate, confidential list of the case numbers and employee names.
Where to Record
Record at the site where the incident occurred. If the incident occurred off-site, then record at the work site of the employee. If the incident occurred in the home of an employee who works at home, record at the employee’s supervisor’s worksite.
