Use this checklist to double check your artwork before submitting.
You may fill in the PDF electronically or print it out and fill it in by hand. If your local PTA or PTSA is using a digital form, use that form.
Submit the entry form with your artwork to your Local PTA or PTSA Reflections Chair.
All required fields must be completed for the category and division entered.
