Candidates must submit a Clinical Experience Verification form for Clinical experiences conducted as the requirements for education courses. One form should be completed for each EDUC course. Candidates should follow the directions provided by the course instructor when completing Clinical Experience Verification form for each specific course.
You will be required to sign in using your Nicholls Student email address in order to access the form. Once a form is submitted, you will receive an email confirmation with a copy of your submission. You may use this email to provide proof of submission to your course instructor. If you need to make any changes to your submission, there is a link to do so from the email confirmation.
Tip: Instead of deleting the confirmation emails, create a folder in your email to save all of the email confirmations you get once you complete a form. You will be able to easily scroll through the confirmations to see the clinical experiences you have already completed a form for.

Edit or Save Responses
Locate the email confirmation you received after submitting a form.
Click the 'edit responses' button to edit any of your responses.
To save your responses as a PDF, click the print button. In the print dialog window, change the destination from your printer to "PDF" and click save.
The document will be saved to your computer. You can upload this document to Moodle.Â
