Important Information...

Please speak with your counselor before starting your permission form if you are planning to take classes for high school credit
- Your counselor will advice you on which classes to take
Remember that Ohlone has strict deadlines. Try to start on the permission form ASAP.
Make sure EVERYTHING is spelt correctly.

Permission form will be available on October 29th at 4pm!!!

Step by Step video on how to complete permission form

Carefully make sure every word are spelled correctly. This is especially important for the emails, as your parent and counselor will be required to sign them. If the spelling is incorrect, you will be required to complete a new form.

Be aware that your guardian will receive an email notification following the submission of your permission form by you (student). The parent is required to create an account, complete the parent section, and submit the form. Upon the parent's submission of the form, your counselor will receive an email and will either approve or deny the form, as well as sign it. Ohlone will begin the application processing once all parties have signed the form. The duration of this procedure may vary from three to ten days. Please be patient.
