Please complete your COVID-19 Health Screening prior to coming to campus. If your answer is "YES" to any question, do NOT bring your child to school. Call the school office to notify us of his/her absence and the reason. You will also need a doctor's note or proof of a negative COVID test in order for your child to return to school. Testing through the CA Rapid Antigen Testing Program at UCFC is for NHUSD Students and Staff ONLY.
Pre-registration is required and must be done beforehand. You will only need to register once.
Testing hours are Monday - Friday, 7:30a.m. - 4:00p.m. at the Union City Family Center: 725 Whipple Rd., Union City, CA 94587. These times are highlighted in yellow on the calendar below.
All results are shared electronically with the contact information provided during registration.
If you need assistance with registration or have questions about the CA Rapid Antigen Testing Program, you can contact the call center representative Monday - Friday, 8:00a.m. - 5:00pm. at (650) 275-5419.
National Labs currently offers COVID-19 testing to Students, Staff & and Family Members. Testing is offered in the Performing Arts Center at James Logan High School, which is on the corner of Alvarado-Niles Road and H Street.
Appointments not required and testing is available Monday - Friday, 10:00 a.m. until 6:30 p.m.

It is recommended that all COVID-19 test recipients fill out, sign, and bring the initial intake and consent forms linked below, with them to the testing site.
