Welcome! We're so happy you've joined us here at Eaton Elementary School.

This page contains all of the necessary School Health documents to you get started. If your student has any health concerns, please give me a call at 910- 397-1544 x 71711 or email me at tammy.frieberg@nhcs.net to discuss it with me directly so we can formulate a plan.

Step 1: NC Health Assessment

  • Is your child a rising Kindergartener, or is he/she transferring to us from out of state?

    • If so, please have your child's physician complete a North Carolina Health Assessment within 30 days of enrollment. ( see the form above).

    • This is a state law. Students will not be allowed to attend school if this form is not received by day 30 of enrollment.

  • If your child has severe allergies, asthma, or diabetes, please see the Forms page, the Emergency Care Plan page, and the Resources page for more information.

Step 2: Immunization Records

  • Please provide an up-to-date record to the front office of all the immunizations your child has received so far, within 30 days of enrollment.

  • North Carolina law requires all public school students to be in compliance with the CDC's recommended vaccine schedule, or to provide a medical or religious exemption letter.

    • A copy of the Medical Exemption from Immunizations form can be found on the Forms page.

    • If your child isn't currently in compliance with the CDC's vaccine schedule and doesn't have an exemption, you will be required to show proof of participation in a vaccine catch-up schedule.

    • Failure to comply with the vaccine schedule may result in school consequences.

STEP 3 HEALTH ASSESSMENT (physical exam)

Every new student in New Hanover County schools MUST have a recent ( less than 1 year old) physical or Health Assessment on file within 30 days of their first day in school.
