- Interscholastic Athletics Parental Consent Form
Must be original copy, handwritten, in pen (cannot accept a emailed, printed, or faxed copy)
All 13 statements must be read and understood by the parent/guardian and initialed in pen (2+ letter initials)
Can only list 1 sport per form (ex. CANNOT have Soccer/Basketball, fill out a separate parent consent for each sport)
Must be signed and dated during the current school year (Between July 1st. and June 30th of the current school year)
Please note that this form is designed for legal paper, change the printing to fit onto letter paper
2. Interscholastic sports medical form
History form is to be completed by the patient and parent before seeing the medical provider
All pages INCLUDING the PSAL Health History COVID Addendum is to stay in the child's medical file with the doctor, EXCEPT...
The "Recommendations for Participation in Physical Education & Sports" form is the ONLY page turned into the coach/AD/school
The "Recommendations for Participation in Physical Education & Sports" form must be original copy, in pen from the medical provider
The medical provider can fax/email the form directly to the coach/AD/school but cannot give to the parents prior to the school receiving it
The "Recommendations for Participation in Physical Education & Sports" form must include the medical provider name, signature, license number, and medical stamp.
3. Concussion (Student and Parent Information Sheet)
All student-athletes and families must read over the concussion information sheet prior to participation and be aware of any signs of a concussion
