Disqualifying conditions

The following conditions are disqualifying for Summer Leadership Camp due to the lack of available emergency facilities.

Any history of asthma, even if controlled through medication.
Current use of an inhaler with or without asthma.
Any history of heart problems.
Any history of seizures, even if controlled through medication.
Cadets that are pregnant.

CURRENT Sports physical is REQUIRED

Must be signed by a medical professional no earlier than one year from the last day of camp (Saturday graduation).

Health Risk Screening Questionnaire

For NJROTC Summer Camps, this form must be signed no earlier than 90 days prior to the end of camp.

If ANY blocks are marked "YES" the form must be signed by a medical professional.
