Required forms

The forms below are required for all cadets to attend NJROTC Summer Leadership Training Camps.

All forms should be returned to the cadet's NJROTC Instructors. They will register them for camp.

1. Sports Physical

All cadets need a sports physical dated within one year of the end of camp that clears a student to participate.

The form can be a district or state approved form.

The sports physical does NOT replace the Health Risk Screening Questionnaire below. Both are required.

SRF

2. Standard Release Form

Form must be signed within 90 days of the end of camp.

- Doctor and Health Insurance information on the back page is mandatory.
- Dental Insurance is optional, however the information provided may be required for non-emergency care.

HRSQ

3. Health Risk Screening Questionnaire (HRSQ)

Form must be signed within 90 days of the end of camp.

NOTE: If any question is marked "YES," the form must be signed by a doctor.

The HRSQ does NOT replace the sports physical. Both are required.

Indemnity

4. Camp Shelby Hold Harmless

Required for all persons training at Camp Shelby

- Parents should sign and date as the Individual
- Instructors should sign and date as witness
- Parents should enter emergency contact information

Cadet information

NJROTC Unit instructors will need information to register cadets for the camp. Contact your Naval Science Instructors for specific information required.

For routine questions before or during camp, parents should contact the Naval Science Instructors at the cadet's school.

Information of a non-emergency nature should also go through the Naval Science Instructors at the cadet's school.

Note: Cadets are prohibited from meeting any visitors at any time between drop-off and graduation.
