Insurance Benefits
Overview
How often do you think about insurance? Probably not too often unless you've just paid for it or had a sudden need to use it. Keeping track of what insurance benefits the company provides, which you are signed up for automatically, and which you have selected voluntarily can be a challenge. For specific information about your benefits selections, click here for your Zenefits login. Below is a brief explanation of the insurance benefits available to you, as well as some helpful resources on the plans that we currently offer.
Mandatory Coverage:
National Flatbed provides dental, vision, life, workers-comp, short-term disability, and long-term disability insurances to all employees. The company pays 100% of these premiums, and participation in these plans is *mandatory. Obviously, you don't have to use the insurance (and in some cases, we hope you won't need to!) but you do have to be signed up so that you have the coverage if needed.
As it has been explained to us, if we pay 100% of the premiums then 100% employee participation is required. We're not just being pushy here, but if we are going to pay the entire premium for those who are interested, then we have to for everyone. Even if you have dentures and don't want dental insurance, etc.
Voluntary Coverage:
National Flatbed provides a couple of options for health insurance:
1) The company will pay 50% of the policy premium for the AZ Banner Perf Silver OAMP 7150 80/50 (for 2022/2023). If you choose a different policy, the company contribution is still one half of the premium of the plan previously mentioned. The match amount is different for different employees since age influences the cost of insurance premiums. So, the longer you stick around, the more the company pays to help off-set the cost of your premium!
2) For employees who do not want or need the company-offered health insurance (for a variety of reasons, including receipt of VA benefits, tribal benefits, coverage through a spouse, etc.), the company offers a cash-in-lieu of benefits option. This benefit is $200.00 per month, applied as $100.00 per pay period. The goal of the cash-in-lieu of benefits option is not to disincentive use of the company-offered plans, but to ensure that everyone is receiving similar company benefits, even if they have health coverage from a different source.

98point6
98point6 is on-demand, text-based primary care that works with all of our currently offered Aetna plans.
To use 98point6, you and your adult dependents simply need to download the free app (click here for the Apple App Store or here for the Google Play Store) and create a profile. This only takes a few minutes and will require your name and, if you have an HSA plan, credit card information. Most of our employees are not on HSA plans, but if you have any questions about what kind of plan you are enrolled in, please email admin@nationalflatbed.com.
All 98point6 visits are offered at no additional cost to the you and your covered dependents, unless you have a qualified HDHP HSA health plan. HSA plan members will be charged a flat $5 fee per visit.
Important details:
1. 98point6 automatically replaced Teladoc effective 7/1/19 - no action was required
2. All visits are with U.S.-based, board certified physicians licensed in all 50 states
3. Use of these services falls within the scope of primary care visits
4. The 98point6 app is downloadable from the Apple and/or Google Play stores
