Physical Examinations & Immunizations
Natick Public Schools, in accordance with Massachusetts Department of Public Health 105 CMR 200.000 and MA 105 CMR 220.00 regulations, requires a physical examination and certificate of immunization before the student enters school and before entering grades K, 1, 4, 7, and 11.
The physical exam should contain evidence of:
an up-to-date immunization record.
written verification that the student is fit for all activities.
the examining healthcare provider’s written or electronic signature, and
date of exam.
The physical exam for entrance to kindergarten should also contain evidence of:
vision screening with stereopsis.
TB risk assessment.
lead screening.

Immunization Exemptions:
If there is an outbreak of a communicable disease (which has a vaccination mandated by Massachusetts DPH), those students with exemptions shall be excluded from school and all school sponsored activities for the duration of the incubation of that particular illness as outlined under under 105 CMR 300.000 reportable diseases, surveillance, and isolation and quarantine requirements.
Medical Exemption - if there are medical reasons why your student has not been immunized, a letter must be obtained from your healthcare provider and given to your school nurse; this letter shall be provided annually at the beginning of each year.
Religious Exemption - if there are religious reasons why your student has not had a physical exam and/or has not been immunized, a Religious Exemption form must be provided to the school nurse, signed by the student’s parent/guardian and attest that the immunization(s) are against one's sincerely-held religious beliefs. These form letters shall be submitted annually, at the beginning of each school year.
