Sport Requirements

All middle school athletes must complete the following before their respective sport season begins:

1. Have a sports physical. This must be completed once a year by a certified medical professional. Please turn in your signed physical form to your school office immediately.
   * Grades 6 and 7 - You must have a new sports physical for the 2020-2021 school year.
   * Grade 8 - We only need Physical Form B turned in this year if you had a physical last year. The for,m can be found below.

2. Pay your participation fee.

3. Complete the MHS Athlete Registration Form. This must be done for each individual sport you participate in during the school year.

Sports Fees

Please pay the following fees in the Jim Hill office before the season begins. If financial assistance is needed, please contact the Jim Hill office.

1st and 2nd Sport = $25.00 Each

3rd Sport = $12.50

4 or more sports = FREE (Maximum fee of $62.50 for all middle school athletes)

Sports Physicals

All middle school athletes (Grades 6-8) are required to have a sports physical completed before being allowed to participate in any and all activities. When completed, the physicians form must be turned into the school office immediately.
