Sport Information

Please use this website as your main source for Jim Hill sports information. Please scroll to the bottom of this page for important information. In addition, the following must be completed for each season.

1. Sports Physical - All athletes must have a sports physical current for the 2020-2021 sports season. Turn this form into your school office. This must be completed once a year by a certified medical professional. Please turn in your signed physical form to your school office immediately.
* Grades 6 and 7 - You must have a new sports physical for the 2020-2021 school year.
* Grade 8 - We only need Physical Form B turned in this year if you had a physical last year. The for,m can be found below.

2. MHS Athlete Registration Form - Click this link to fill out a required form for all MPS athletes. This must be completed for each sport they participate in.

3. Pay your participation fee to the office.
