Schedule Change Request Form
This form is for students requesting a change to their current class schedule. Please note that not all schedule changes are possible and may depend on course availability, graduation requirements, schedule change policies, and administrative approval.
Before submitting, be sure to speak with your parent/guardian about your request. Counselors will review submissions in the order they are received and follow up via email or in person as needed.
Transcript Request Form
Whether you require it for employment purposes or higher education, you can conveniently request your High School Transcript here.
Appointment Request Form
Complete the form to request an appointment with your school counselor. Your request will be sent to the counseling office secretaries, who will schedule your meeting based on both counselor and student availability. Please do not use this form for emergencies! If you are in crisis or need immediate support, go directly to the counseling office or tell a trusted adult.
Release of School Records Form
This form grants permission for Miami Trace High School to release a student’s educational records to a designated individual, school, or organization. A parent/guardian or eligible student must sign the form before records can be shared.

Work Permits
1. Print a Work Permit Application here or pick one up in the Main Office.
2. Fill out the top section with a parent.
3. Have your employer fill out the bottom section.
4. If you don't have a Physical already on file for sports, print out a Physical Form or pick one up in the Main Office. If you are having trouble getting a physical, please contact your school counselor.
5. Once all steps are completed, visit Mrs. Bryan in the Main Office to get your official Work Permit.

Dublin Driving School
We will be holding driver's education classes in the Miami Trace Learning and Wellness Center. Our first class begins in January 2026, and there are a limited number of spots available. Please register and submit a $50.00 deposit to secure your seat.
