[INSTRUCTIONS: If you want to include a contact form or a way for visitors to leave comments, one option is to embed a Google Form. A sample form is embedded below. Please make a copy of the form and then click SEND in your new form to get the embed code. If you double right click the form below, it will give you a popup box where you can replace the embed code with the code for the form that you own.
In this version of the template, the Contact Me page is under Home and shown in navigation. Also, there is a button in the Footer that connects to the Contact Me page. You can also do any or all of the following:
a) Use a link from the footer or the home page directly to the Google Form rather than a separate page in the site.
b) Hide the Contact Me page from navigation - this is a More option next to Contact Me under Pages.
c) Just give people your contact info on this page.
d) Don't use a Contact Me option at all. It's all up to you!]
