As a new student SWHS requires additional documents if they had not already been filled out during the online enrollment process. Below is a list of what we need in order to complete registration. Please fill out then send to southwest.counseling@mpls.k12.mn.us before you come in to choose classes. If you do not have access to a printer, we will have paper copies for you to fill out when you arrive. Please plan on additional time after you meet with your Counselor to stay and complete the forms before you leave. Registration is not complete until all of the information has been received. A copy of the student’s Immunization Records or Conscientious Objection Form
WIDA Access Score Language Testing/ELL Records (if applicable please provide a copy)
Indian Student Eligibility (only need this form returned if student is eligible)
A copy of Student’s Birth Certificate, Passport, State ID, Baptismal Certificate, or other legal documentation for name and birth date verification
504 or IEP records (if applicable please provide a copy)
