This document outlines a facilitation plan for a meeting where families give input on your school's FEP. Templates are available to support you in documenting this meeting:
Meeting agenda (Hmong, Somali, Spanish versions are available)
Meeting slides (Hmong, Somali, Spanish versions are available)
Planning Worksheet for Your FEP
Use this worksheet to complete your FEP before entering the information into the SIP portal. If the staff member who is responsible for completing the FEP does not have SIP portal access, they can complete this worksheet (based on family input) and give it to someone who does have SIP portal access to enter.
