When Consultation Services are considered:
Students are placed on the consult model to monitor the maintenance and mastery of the given skill that was targeted in speech.
It is NOT necessary to wait until a triennial to exit a student from speech and language, as long as you gather appropriate assessments. Out of consideration for the team, if you have a student meeting their goals that you know is a candidate for being exited from speech and language and they have an upcoming Annual Review, work with the case manager to generate an assessment plan to determine eligibility for services .
If the parent disagrees with discontinuing Speech and Language services, follow up with the site PM to determine the next step in the process.
The SLP should be attached to an established classroom goal as a service provider.
Note: goals drive service, and if you are placing a student on a consult model a speech-only goal wouldn't typically be necessary.
'Language and Speech (415)' Services should be recommended based on the individual needs of the student in order to meet the targeted goals.
When adding or continuing a service, change the dates of the service to reflect the upcoming IEP year.
Provider should be listed as "120 SELPA" in most cases.
All services should be listed as "session based", include a specific number of minutes, include a specific number of sessions/frequency, with a frequency noted as, "monthly" in most cases.
Most often, sessions would be provided in a 'separate classroom in a public integrated facility (520)'; but may be listed as 'regular classroom public day school (510)' if the sessions are being provided in the general education classroom.
**When recommending sessions during Extended School Year (ESY), you're recommending continuing services during ESY (possibly at a reduced frequency) to ensure that the student does not regress beyond their ability to reasonably recoup their skills upon return to school in August.
These sessions should be listed as "yearly" and include the total number of sessions needed during the 4 week period of ESY. For example, if a student typically receives "6 sessions monthly", a, you might propose "3 sessions, yearly"
As a starting point when recommending the frequency of sessions:
one session weekly, 3 sessions per month
two sessions weekly, 6 sessions per month
three sessions weekly, 9 sessions per month
