Rosters are to be completed on a monthly basis. The date range for the rosters can be found on the SEIS homepage.

Make a copy to keep for yourself
Send the original to SPED dept (SEIS) by the Friday after the last day on the register
SLPA Rosters can be completed in 1 of 2 different ways:
On the SLP's roster
On a separate roster
The SLPA will sign the roster
The SLP will sign the roster and send to District Office with his/her own roster
Supervision of new SLPs (CFY/RPE Supervision)
SLP Supervisor is responsible for
ASHA
A supervisor can supervise up to 2 people in their Clinical Fellowship Year (CFY) at a time.
Supervising someone during their CFY requires 6 hours of direct support every 3 months.
This includes visiting the site of the CFY and observing their day to day activities.
Supervising a CFY requires 6 hours of indirect support every 3 months.
This includes video observation (for example SKYPE) of the CFY as well as review of documentation (reports, etc.) and support that does not involve direct observation.
CA State License
No more than 3 SLPs earning their Required Professional Experience (RPE) at once can be supervised by the same SLP.
8 hours of direct supervision per month (4 hours for part-time RPE)
This includes direct observation of the SLP in therapy, during IEPs, etc.
SLPA Supervision
Each SLP can supervise up to 2 SLPAs at a time.
Responsibility statement for Supervisory of a Speech-Language-Pathology Assistant
Submitted within 14 days from start date of supervision to ASHA. If it is the beginning of the school year, paperwork must be submitted within 14 days from the start date of the year.
1 lead supervisor will be designated but ALL SLPs working with the SLPA must fill this out and submit to ASHA.

Termination of supervision for SLPA or RPE
Must be submitted to ASHA within 14 days from the date that the supervision was terminated

Data collection is a vital part of our job as SLPs. It allows us to accurately track and modify therapy plans on a regular basis to ensure that our students are receiving the best intervention possible. Below are several examples of therapy logs for you to use or get inspiration from. Please feel free to use whatever therapy log you prefer.
Therapy logs are also crucial for documentation and proof of service. Data collecting and tracking serves as a way to ensure that you as the service provider have proof of your work.
