• All schedules/dates need to be submitted through the submission form. Email requests are no longer accepted. 

• Any/All additional dates not originally listed need to be submitted through this Submission Form. All required forms/documents must be submitted with each request, each time, no exceptions.

• You will receive a confirmation email with dates that were confirmed or not available. 

Valid Certificate of Insurance & Valid CPR cards are the responsibility of your organization. 

*Please submit your COI and CPR card updates before they expire to avoid being removed from the schedule. Reminder's no longer get sent out to renew any out of date COI & CPR cards. 

Certificate of Insurance & CPR Card Submission Only:

If you are in the middle of your contracted dates during the school year and your COI or CPR cards expire, they need to be submitted prior to expiration through the submission form below. 

Certificate of Insurance & CPR Card Submission 

If you submit an up to date Certificate of Insurance or CPR card the day of your scheduled event after 3:00pm, you will not be able to use the building until the following business day when it can be confirmed.
