Hello Parents. Please complete this form when referring students to the school counselor. At times your student may experience times of transition, heightened stress, critical change or other situations impeding student success. When this occurs, your student may benefit from counseling. Counseling in the school setting is short-term and based on counseling theories and techniques that are effective in a school setting to promote academic, career and foundational wellness. When a student requires long-term counseling to provide long-term support after a crisis, the school counselor will collaborate with families and other professionals and make referrals to appropriate community resources.
