Please read these instructions carefully, and fill out this form if you believe there is an error in your MHS schedule. Submissions that do not pertain to an error will not be considered.

Applicable errors include:

- You are scheduled for a class you have already taken and passed

- You are scheduled for a class and have not taken the necessary prerequisite(s)

- There is a hole in your schedule

- You would like to increase your academic rigor (eg. move from regular English to honors English)

- There is a timing conflict between your MHS classes and in-person CCP courses

We are not able to accommodate schedule changes unrelated to scheduling errors, and cannot make changes to schedules based on student preference. If you are scheduled for your first choice and/or alternate electives, as well as classes needed for an on-time graduation, your schedule will remain as is.

Once you complete this form, your assigned counselor will follow up with you. 

You will be contacted in the event a change is made. Schedules will not be changed back once a change has been made.
