SENIOR END-OF-YEAR CHECKLIST

From the School Counseling Center

Please make an appointment with your Counselor (McCormack A-G, Harkins H-M, Scott N-Z) for more information or assistance. 

• If applicable, file and complete all Financial Aid.

• If applicable, log where you will be attending school in Naviance.

• Complete the Graduation Survey AND the MHS Senior Survey in Naviance. Access information can be found here.

• Pick up your Medway School District student folder at Graduation Rehearsal. You must sign the acknowledgement form!

From the Nurses’ Office

Please email or check in with Mrs. DiSalvio with any questions or concerns. Students under 18 may need a parent to pick up medication on their behalf. 

• Pick up your medical file at Graduation Rehearsal. You must sign the acknowledgement form!

• Meet with the Nurse to formally collect and acknowledge receipt of any medication(s) or medical device(s).

From the Athletics Department

Please reach out to Mr. Parcells or Mrs. Culcasi with any questions. 

• Student athletes must return all uniforms or equipment

From the IT Department

Please email support@medwayschools.org with any questions.

• Return chromebook and charger in good, working condition. If items are lost or damaged, student is responsible for settling any debts with the IT Department and Administration

From the MHS Teachers, Department Heads, & Academic Supports

For Department specific support please reach out to: 

Cecchi (English), Macedo (STEM), Swenson (Math), Pacelli (History), Dr. M (World Lang.) or your teacher. 

• Return all loaned classroom supplies, including but not limited to; textbooks, reference books, literary novels, calculators, lab materials, etc. to the appropriate teacher

• Return all library books to the School Librarian, Mr. Sullivan.

From the Administration, Main Office, & Food Services

Food Service Debts can be paid online or in the Kitchen. Class dues can also be paid online. Any other concerns should be directed to Mr. Augusta. 

• Resolve any disciplinary consequences, including detentions for tardiness

• Resolve any debts to the school and/or cafeteria

• Payment of class dues
