The FRC or Final Requirement Checksheet is the form that an educator uses to show the LPDC committee how their professional development allows them to renew their teaching certificate/license.
Each staff member must complete an FRC, and include the supporting documentation and/or transcripts for their professional development activities that they are using to renew their current license.
Once the LPDC committee approves the FRC, the educator will be sent a letter outlining the final steps that are required for sending your license renewal to ODE.
You will complete the license renewal process by applying for your new license with ODE.
When completing the ODE application, you do not have to download any of your materials for renewal on the application (you are doing this with our LPDC). In the signature line, you will use the search for LPDC tool. Our LPDC name is Maumee City LPDC. This will send a notice to me stating that your application is ready for my verification.
