Students may earn graduation credit by generating and developing an Individual Plan for any of the following options: independent study, internships/field experiences, or educational travel. This also includes a Physical Education credit flex plan.

FIND A MENTOR
OR TEACHER OF RECORD
All individual plans require a mentor. A mentor is someone who is responsible for assisting the student with their individual plan and can assess the student’s progress on the plan and/or be contacted by the credit flex administrator. Mentors cannot be related to the student and should be over 21 years of age.
In order to complete an Individual Plan, students may be required to have a licensed Ohio educator to serve as Teacher of Record (TOR). This individual must meet teaching licensure standards and requirements for core courses as determined by the Ohio Department of Education. The student will be notified if this is a requirement of the individual plan after meeting with the credit flex administrator.
For Physical Education Individual Plans, a mentor could be an instructor, coach, trainer, etc.
REVIEW PREREQUISITES IN THE COURSE OF STUDIES
Students must finish any current prerequisite course before beginning another course.
Example: Algebra I must be completed before taking Geometry. These courses cannot be taken at the same time.
If the online course serves as a prerequisite for another class, it must be completed before a student can be scheduled into the next course in the sequence.
Example: Algebra II (online) must be completed before enrolling in Pre-Calculus at MHS.
THINK IT THROUGH AND DISCUSS WITH YOUR FAMILY
The decision to pursue credit flex is a family decision and should be student-driven. It’s important to consider how it will fit into the student’s weekly schedule and how to maintain a healthy balance with other commitments.
Before submitting an application, must be able to commit to the Credit Flex Completion Deadline.

STEP 2: APPLY, COMMIT, & PLAN AHEAD
SUBMIT THE CREDIT FLEX APPLICATION ONLINE
Submit the Credit Flex Application within the application timeframe. It is important students submit the application from their MHS email account- not their personal account.
Students will need to provide specific information regarding their individual plan. For details, review the Credit Flexibility Handbook.
WAIT FOR THE OKAY
BEFORE YOU START
Do not begin until your Credit Flex plan has been officially approved by Mr. Powell.
Students will receive an email indicating if the course was approved or not within 2 weeks after they submit the Credit Flex Application.
RETROACTIVE CREDIT WILL NOT BE AWARDED UNDER ANY CIRCUMSTANCES.
All costs related to individual plans are the responsibility of the student/applicant.

COMPLETION DEADLINES
Students on approved Individual Plans may be periodically required to provide evidence of progress and attendance to the credit flex administrator.
Students failing to meet the standards for ongoing participation and satisfactory progress described in an Individual Plan may be reassigned to traditional courses at the discretion of Mason City Schools.
Students should create a plan with their mentor to ensure they will meet the Credit Flex Completion deadline.
Summer Plans - Due by August 1
First Semester Plans - Due by December 1
Second Semester Plan- Due by May 1
STEP 3: SUBMIT CREDIT FLEX COMPLETION FORM
Upon completion of the plan, students are required to submit the CREDIT FLEX COMPLETION FORM, and also upload evidence of completion BEFORE THE DEADLINE in order to receive a grade and credit.

PLAN SUBMISSION REQUIREMENTS
ACTIVITY LOG
Record the type of activity, date, and duration for each session.
Total your hours at the bottom of the log.
Have your mentor sign off verifying that you completed the listed activities.
A minimum of 60 hours of physical activity is required to earn 0.25 credit.
WRITTEN REFLECTION/EVIDENCE
Provide a reflection and/or supporting evidence explaining how your Individual Plan aligns with the five Ohio Physical Education Standards for grades 9–12.
