NURSE
Students who are feeling ill or have a health related concern should report to the nurse’s office.
The district DOES NOT provide an accident insurance plan for students. An accident plan is available at the expense of the parents. Accident forms must be obtained at the main office and returned as soon as possible. If you become ill or injured while in school:
Please notify a staff member or administrator of the illness or injury.
If an illness or injury requires immediate care, the office staff will aid you in every possible way.
State Statute (140.05) requires each student to present written evidence of completed basic and booster series immunizations; unless a written waiver to the School Board within 30 school days after being admitted to school. Personal hygiene products are available in the guidance department, physical education and team locker rooms.
MEDICATION
Parents shall notify the principal in writing whenever a student is required to take medication per administrative guideline.
It is the parent/guardian's responsibility to see that a student receives any needed medication or procedures until the requirements in this policy are met.
No medication or procedure will be administered to a student without the written authorization of the parent/guardian.
Medication must be provided in the original unopened and sealed container with the manufacturer's list of ingredients and instructions for use.
THE DISTRIBUTION OR CONSUMPTION OF PRESCRIPTION OR OVER-THE-COUNTER MEDICATIONS IN ANY MANNER OTHER THAN DESCRIBED ABOVE IS A VIOLATION OF SCHOOL RULES AND MAY BE IN VIOLATION OF THE LAW. CONSEQUENCES MAY INCLUDE EXPULSION FROM SCHOOL AND REFERRAL TO LOCAL LAW ENFORCEMENT.
