Participation Forms & Invoices

Participation Agreement - Please have a participation agreement for all students auditioning. This year we will be submitting Participation Agreements electronically via Google Form. Please submit Participation Agreement forms here. 

Audition Invoice - Please send this form with payment to Zach Lampron on or before the audition date.   

• $10.00 per audition; non-refundable. 

• Please send one check, payable to MMEA District II, to cover all students from your school. 

• Prior payment is preferred, but you may bring a check with you to auditions.

• We have a "No Pay/No Play" policy.

Acceptance Invoice - Please send this form and its payment to current treasurer by the acceptance deadline.  

• $20.00 per HS/MS student; non-refundable

• Please send one check, payable to MMEA District II, to cover all students from your school. 

• "No Pay/No Play"

Please upload your elementary participation agreements to this Google Folder by March 31st, 2026.

Checks and invoices should be mailed no later than March 22nd or brought in hand on April 10th. Thanks!
