Halfway through NUR 323/325, University of Southern Maine (USM) nursing students are eligible to become certified as a nursing assistant (CNA) in the state of Maine.

Students, please follow these steps to complete the process:
Ask the instructor of your NUR 325 clinical course (or current clinical if you are past 325 in the curriculum) to complete the Mid-Semester Clinical Evaluation for CNA Registry Eligibility form. Only faculty should fill this out, not students.
After your faculty completes the form, it will automatically be routed to Lindsay Larsen.
If you are passing NUR 325, then Lindsay Larsen will write a letter for you to include with your application to the CNA Registry and email that letter to you.
Gather your documentation:
Copy of your driver's license or passport
Letter from the School of Nursing provided by Lindsay in step 2
Copy of your criminal background check report from Verified Credentials
Complete the Maine CNA Registry Application from the DHHS website using the documentation you have collected.
