Below are the state of Massachusetts health record requirements for entrance into school

At the bottom of the page are links for blank immunization and physical exam forms to print and take to your primary care provider if needed.

Immunizations

According to Massachusetts General Law (M.G.L. c. 76, ss 15 and 105 CMR 220.000), a child is not permitted to enter school without evidence of up to date immunizations.* The immunizations needed are as follows:

Preschool

3 doses of Hepatitis B

4 doses of DTaP/DTP

3 doses of IPV/OPV

4 doses of Hib

1 dose of MMR

1 dose of Varicella*

Kindergarten

3 doses of Hepatitis B

5 doses of DTaP/DTP

4 doses of IPV/OPV

2 doses of MMR

2 dose of Varicella*

DTaP/DTP = Diptheria,Tetanus, Pertussis

OPV/IPV = Oral Polio/Inactivated Polio

Hib = Hemophilus influenza type B

MMR = Measles, Mumps, Rubella(German Measles)

Varicella = Chicken Pox *(or physician certified history of the disease)

*Medical exemptions (dated statement signed by a physician stating that a vaccine(s) are medically contraindicated for a student) and religious exemptions (dated statement signed by a student or parent/guardian, if the student is < 18 years of age, stating that a vaccine(s) are against sincerely held religious beliefs) must be submitted annually, at the start of the school year.

*Students who are not immunized may be excluded from school if exposed to another student or staff member with a vaccine preventable disease. Click the link below for more information on this:

Lead Screening

Massachusetts law and regulations (105CMR 460.050, and .060) requires periodic blood lead screening for preschool children. It also requires that children present evidence of having been previously screened for lead poisoning for entry into kindergarten. (Test may be from any time in a child's life.) ( see below for regulations)

Physical Examination

A periodic physical examination is important for all children and adolescents. The physical examination is crucial for preventative, diagnostic, or corrective purposes. Massachuseets General Law (M.G.L. c. 71, s.57 and 105 CMR 200.000-200.920) require physical examinations of school children before entry into school and for kindergarten, fourth grade,seventh grade, and tenth grade.
