If your child has a life-threatening allergy, it is important to have their life saving medications at the school.  Please make every effort to bring in appropriate medications and an Allergy Emergency Care Plan to leave in the health office.  Each school year a new allergy plan needs to be filled out with new medication orders from your child's physician. A bus card should also be filled out so we may share the allergy with the student's bus.  Copies of both of these forms are below.
