Participant Code of Conduct
General Rules:
Participants shall be respectful of all others, which includes teachers, staff members, bus drivers
and other students.
Participants shall follow directions from any and all staff members
Prohibited Behaviors:
Bullying or acts of aggression or violence.
Continual disruption of the program.
Refusal to follow the behavior guidelines.
Inappropriate physical contact
Yelling or screaming.
Using profanity or any inappropriate language.
Displaying clothing or other items with offensive content on it.
Stealing, damaging, or failing to care for CYA or personal property
Bringing unauthorized electronics to the program -- participants shall leave all electronic devices at home such as iPods, hand held computer games, and cell phones. (CYA is not responsible for any lost or stolen electronics.)
Possession of weapons-any object that may cause harm to another, or place another person in fear of his/her safety, may be considered a weapon.
Students Rights and Responsibilities including Internet & Technology:
Please be reminded, Community Youth Advance adheres to Prince George's County Public Schools administrative procedures regarding appropriate use of technology, social media and email which continue to apply to online instruction. A description of the applicable procedure is provided online in the PGCPS Student Rights and Responsibilities Handbook.
Steps Taken After Failure to Follow Behavior Guidelines:
First Offense:
Staff will redirect the participant to a more appropriate behavior.
Second Offense:
Staff will discuss the problem with a parent/guardian and prepare a written incident report outlining the problem and corrective action.
Third Offense:
Withdrawal from the program. CYA will not refund any registration or activity fees, if applicable, due to Code of Conduct violations.
Egregious Violations:
All egregious acts will result in immediate withdrawal without warning and/or parent conference. This includes, but is not limited to, any student’s behavior that at any time threatens the immediate safety of themself, other participants, or staff. In the case of immediate withdrawal, the student’s parent/guardian will be notified and expected to pick-up the child immediately. CYA will not refund any registration or activity fees, if applicable, due to Code of Conduct violations.

I have enrolled my student in a Community Youth Advance (CYA) out-of-school-time program and authorize my student’s school to release the below named educational records so that CYA may track, manage and seek to improve student performance:
Attendance Records
Transcript
Report Card & Progress Report Card Grades (All Subjects)
State and Country Assessments
Disciplinary / Behavior Records
Special Education / 504 / IEP / ESOL Records
State Student ID#
Any other material pertinent to the growth or needs of the student in an out-of-school-time setting.
Media Release
Throughout the program, Community Youth Advance, via our partnership with Prince George's County Public Schools, will conduct activities that may be publicized through local or national news media. These activities may include interview sessions with news reporters; photographs of individual students or groups of students for newspapers or various school system publications including newsletters, calendars, and brochures; the use of student photos on the Community Youth Advance web site; and videotaping for local and national television news programs, cable programming, and school system promotional videos.
*Students will only be identified by their first name unless the reference to the student is in a manner necessary to use their full name.
