Makeup Forms

PE class makeup forms can be picked up in the gym at school or printed and returned from the link below. All makeups are done outside of school, require a parent/guardian signature and can be any type of physical activity ranging from:

• Organized sports team practices/games

• Recreation sports

• Cardiovascular activities (walk, jog, run)

• Weight training or other gym activities

• Skating, biking, surfing, hiking

• Swimming, dancing, yoga, gymnastics

• Rule of thumb -- ANYTHING THAT MAKES YOU SWEAT!
