School personnel may not administer medication without an accompanying physician's note on the LUSD medical authorization form. This policy includes aspirin, cough syrup and other over the counter remedies. Students are not permitted to keep medication of any kind in their backpacks or in their classrooms. All medications are to be stored in the nurse's office and are to be administered only when the physician's note and completed authorization form are on file. All medication brought from home must be in the original container (this includes over the counter medications). Medication authorization forms are available in the school office.
All medication orders will expire at the end of the school year, regardless of the date they were issued. Medication orders must be renewed at the beginning of each school year (dated AFTER the last day of school from the previous year).
