Medication

Longmeadow Public School nurses are allowed to administer some over-the-counter medications such as Acetaminophen (Tylenol) and Ibuprofen (Advil or Motrin) with annual review by our school physician. At the beginning of each school year, parents/guardians will receive the Confidential Medical Information Form to sign which gives the nurse permission to administer these medications. The medications are kept in stock in the Health Office with the exception of cough drops. Cough drops must be supplied by the parent/guardian. 


If your student has an emergency medication such as an Epi-pen or an inhaler, your student’s healthcare provider must write an order for that medication to be administered and provide an action plan. These forms must be signed by both the provider and the parent/guardian.


If your child needs a daily medication while at school, then your child’s healthcare provider must write an order for that particular medication to be administered in school. The form must be signed by both the provider and the parent/guardian.

All medication brought into school must be in its original, labeled container 


Medication Authorization Form:  This form needs to be completed for prescription and non-prescription medications.  The upper portion needs to be filled out by parents.  The lower portion needs to be filled out and signed by the physician.  Medications that are prescriptions should be brought in a medication bottle labeled with the child's name and medication information.


Confidential Medical Information Form:  This form needs to be completed yearly.  Please make sure to update physician and dentist information.
