Confidential Medical Information Form
This form is to be completed yearly. Please be sure to update physician and dentist information.
This form needs to be completed for prescription and non-prescription medications. The upper portion needs to be filled out and signed by parents. The lower portion needs to be filled out and signed by the physician.
Allergy and Anaphylaxis Emergency Plan Form
This form must be completed for children that have allergies and use an Epipen. It needs to be completed by the physician and signed by both parent and physician.
This form must be completed for those children that have a diagnosis of Asthma and use an inhaler. It should be completed by the physician.
Massachusetts Physical Exam Form
Per MA DPH regulations, all students entering 7th grade need to have an up to date physical on file with the school. Please have your child’s physician complete this form or provide a copy of their physical form
Per MA DPH regulations, all students entering 7th grade are required to have updated immunizations including one dose of Meningococcal vaccine and one dose of TDaP vaccine. Please have your child’s physician complete this form or provide a copy of their immunization record.
