A strong transition plan is built around 3 goal areas:
Future Employment
Post Secondary Education/Training
Independent Living
Once these goals are identified, the goal setting process can occur. Services and activities are then identified in a coordinated manner to assist students in preparing for their future goal attainment.
Students should be asking themselves the following:
Where do I want to work after high school?
What skills do I need to reach this goal?

Transition Services include a coordinated set of activities working together to prepare students for future employment, postsecondary educational opportunities, and independent adult living skills. Many activities may be supported through local outside agencies.

Reporting on activities to support postsecondary goals ensures students are making progress towards the goals, gives schools documentation the goals are being worked on, and supports transition planning for the next IEP. Sharing of progress with parents should occur at regular reporting periods.
