Transition planning is a process that is used to assist a student with a disability in moving from high school into adult life. The process must include specific coordinated activities that the student will participate in, which support their post-secondary goals. Areas included in planning are employment, further education and training, community participation and daily independent living skills. It is a collaborative effort between the school, the student, the family, and community agencies.
Transition planning increases the likelihood of success and independence in adult life.
