Benefits FAQ
Eligibility and timing
All full-time team members are eligible to join the Libertas Health Care Insurance Plans. Enrollment is open to new hires, team members experiencing qualifying life events (QLEs), and during the annual open enrollment period in the fall.
Libertas' annual open enrollment period occurs during the early fall. Please see communication from Lauren Comforti and Ease to review due dates.

When electing your benefits, please make sure to select ‘Submit’ at the end of the plan elections. Additionally, you can download a PDF of the summary of your benefit elections. Ease also has the option of downloading their application on your mobile device where you can pull up your plan elections at any time.
New hires will be eligible to enroll in health, dental, and vision plans as soon as you begin your employment with Libertas College Prep. Your benefits will be effective the 1st of the month following your start date. The deadline to enroll is 30 calendar days from your start date.
If you do not enroll within the first 30 days of your start date, the next opportunity to enroll will be the next annual open enrollment period, unless you experience a qualifying life event (QLE). Please refer to the benefits guide for the list of QLEs.
Drops or changes to your benefit plans can only be made during your first 30 days of employment, during a qualifying life event (QLE), or during Open Enrollment for the following plan year. year. Please note you will have to provide proof of the Qualifying Event in order for the changes to be processed.
Some common qualifying life events include marriage, divorce, birth or adoption of a child, death of a covered dependent, termination of employment or a change in your spouse’s coverage.
Note: Supporting documentation such as birth certificate, marriage certificate, divorce decree, etc. is required for qualifying event changes. This proof must be submitted to Lauren Comforti at lauren@comfortinsuranceservices.com
You must enroll within 30 calendar days from the qualifying life event (QLE) effective date. Your benefits will be effective the 1st of the month following your qualifying life event (QLE) date.
If you do not enroll within the first 30 days of the QLE effective date, the next opportunity to enroll will be the next annual open enrollment period.
You have 30 days from the date of the qualifying life event to add a child to your insurance coverage. For a newborn, that is 30 days from the date of birth. For an adopted child, it is 30 days from the date the adoption is finalized by the courts.
For newborns, a Birth Record from the hospital will need to be submitted. For adoption, a final copy of the court documents signed by the judge in the case of adoption needs to be submitted.
Medical Plans
We have four medical plan options to choose from through California Choice.
Choosing your health plan is a personal decision. Read the Benefits Guide thoroughly and visit the Ease website to help you decide. The Summary of Benefits and Coverage (SBC) for each plan provides an easy-to-understand summary about benefits and coverage.

Ensuring that your provider is in-network and/or finding a trusted provider is very important in your medical care. There is a Find a Provider link in Ease, you can also visit www.calchoice.com/provider-search to research providers or reach out to the Comfort Insurance Team for assistance.
If you do not want coverage under the Libertas Health Plans, you must go into Ease within 30 calendar days of your hire date & waive coverage.
