Final Forms must be completed by BOTH the parent/guardian and the student athlete in order for the student to be eligible to practice. This must be completed no later than March 30th 2024.
Payment should also be made on Final Forms. If you have questions about this, please contact the athletic director Rita Pendexter.

Parent/Guardian must set up a new account.
Once the parent/guardian is logged in to Final Forms, they will click "Add Student" and fill out the information for the student-athlete.
Please use the student-athlete's school email address when registering them and creating their account.
Once the student account is created, the parent/guardian can add them to Middle School Coed Track and Field.

Notes:
Remember that BOTH the parent/guardian and student-athlete must complete and sign ALL forms.
You can also pay the Pay-to-Play fee on Final Forms.
In order to participate at practice or compete at a meet/invitational, you must have a current sports physical on file. This must be completed by a physician and signed by parent/guardian and student-athlete as well. Download a copy of the sports physical and take it to your primary care physician or one of the places listed at the right.

Always remember to call ahead and make sure that the store has a clinic and is available for sports physicals that day.
