Step 1: LEAP Worksheet (link)
This worksheet must be completed by the building psychologist and the EL Coach prior to initiating the ELL Critical Data process. Tier 3 interventions must have been completed and documented.
Step 2: ELL CRITICAL DATA MATRIX INTAKE FORM (link)
Once the LEAP Worksheet has been completed and the the decision to start the ELL Critical Data process has been made, the building psych should complete the Matrix Intake Form.
Directions:
The building psychologist must be the one to complete the form.
Complete all information.
Start a copy of both the Family Questionnaire & Data document and the Final Decision Matrix Google Sheet. You will need to include (share) the links to complete the Google Form's last 3 questions.
Step 3: Matrix Data Checklist (link): Use this resource to check off each task that needs to be completed prior to the Matrix Decision Team Meeting.
