All clinics are located at the LPS Central Office (146 Maple Street). All visitors should use the front entrance.
Please note: We are NOT able to take walk-ins, as the pharmacist needs to know how many doses to bring and pre-verify your insurance coverage. Because of that extra step, we ask that you sign up at least two days before the clinic date.
Where else can I get vaccinated?
A completed consent form. After registering you will receive an email with a link to the consent form for vaccination Please fill out, print, and bring the form to the clinic for your vaccination
For children under the age of 18 a parent does not need to be present, but the child must bring a consent form signed by a parent/guardian to receive a vaccine
Please bring your insurance cards to the clinic. COVID vaccines are free for all regardless of insurance coverage. Flu vaccines are covered at no charge by most insurance. To determine eligibility to receive a flu shot at no charge insurance cards must be presented at the time of the clinic
Note: Uninsured/underinsured residents can get the flu vaccine at a discounted rate of $47 for regular vaccine and $80 for the high dose/adjuvanted vaccine for those age 65

All LPS employees must be vaccinated against COVID-19, unless they have an approved religious or medical exemption. Staff vaccination was first referenced in the memorandum of agreement dated January 2021 (MOA #2). The vaccination requirement was included in the May 2021 MOA (#3) and further clarified in the August 2021 MOA (#4).
Based on MOA #4:
By October 1, 2021, all existing employees must have their first dose of their COVID-19 vaccination and they must be fully vaccinated within one month.
New employees have one month from their date of hire to be fully vaccinated.
If an exemption is granted, the individual is required to submit evidence of a weekly negative COVID-19 test that each school is offering for free this year. If State funding for testing goes away, employees will submit evidence of a negative COVID-19 test at their own expense.
Proof of COVID-19 Vaccination. To establish that employees have received the COVID-19 vaccine, they will need to produce only a copy of the immunization record they received when vaccinated or an equivalent document obtained from the entity administering the vaccine.
