Emergency Health Form / Medical Information and Consent Form
The emergency health form is used as a quick resource for the nurses to reference with parent names and numbers, as well as student medical conditions or allergies.
This form must be filled out to receive medication in the nurse's office.
Physical Exam Form
Students in the following grades require a physical examination.
K, 6, & 11
Pennsylvania's school health laws state that students in grades K, 6, and 11 must undergo physical exams.
6th-grade students should have Tdap and meningitis vaccines documented with their 6th-grade physical exams. These vaccinations are required for entry into the 7th grade.
11th-grade physicals address the 2nd meningitis vaccine required for entry into grade 12.
Students unable to receive a physical exam with their primary care provider will be offered an exam by the school physician with parental permission.
If you choose to use the school physician, please remember that your student will still require the necessary immunizations for the specific grade level.
Dental form
Students in the following grades require a dental examination.
K, 3 & 7
Dental exams are required for grades K, 3, and 7 students.
Dental exams by the student’s dentist are highly recommended, as their dentist can continue with follow-up care as needed. Students unable to receive a dental exam with their primary care dentist will be offered a dental exam by the school dentist.
