Allergies 

If your child has any allergies or sensitivities, please complete the form on this page. 

 It is important to note that a doctor's written order is necessary if a child requires an Epi-Pen. If a student's doctor requires the student to carry an Epi-Pen themselves, this must be clearly stated in the student's order. Otherwise, the Epi-Pen will be kept in the nurse's office.
