Asthma 

If your child has asthma and requires an inhaler, please fill out the following form. 

 It is important to note that a doctor's written order is necessary if a child requires an inhaler.  If a student's doctor requires the student to carry an inhaler themselves, this must be clearly stated in the student's order. Otherwise, the inhaler will be kept in the nurse's office.
